Telecommunications Reporting Worksheet, FCC Form 499-A, Due April 1, 2025

The Federal Communications Commission requires telecommunications carriers and certain other providers of telecommunications, including Voice over Internet Protocol (VoIP) service providers, to annually report on the Telecommunications Reporting Worksheet, FCC Form 499-A, the revenues they receive from offering service. The Universal Service Administrative Company (USAC) uses the historical revenues reported on this form to determine a non-de minimis filer’s final universal service contribution obligation for the prior calendar year. The Form 499-A covering calendar year 2024 must be submitted by April 1, 2025.

Filers use Form 499-A to register with USAC for the first time and, on an annual basis, to report historical revenues from the prior year. USAC is the administrator of the Universal Service Fund (USF), and all telecommunications companies must register with USAC, whether they contribute to the USF directly or through their underlying carriers.

FCC Form 499-A must be completed online through USAC’s E-File system. USAC has provided Instructions for how to file as well as a User Guide for accessing E-File.
